This discussion has been had before and Rowlands John Mark Ministries website has some interesting examples. Our church recently adopted our own covenant which is probably unique for its brevity. It is as follows:

As an expression of my commitment to Jesus Christ, I commit myself to our church and its vision:

a.. To meet together regularly for corporate worship and fellowship

b.. To regularly give time, money and effort

c.. To discover my spiritual gifts and talents, and use them to serve God in and out of the church

d.. To grow and mature as a Christian

e.. To watch over, pray for and encourage the others in the church

f.. To support the leaders, and work in harmony towards common goals

g.. To accept our church’s understanding of church life and baptism.
